Community Health Hub & Pathways Program
The Community Health Hub and Pathways Program is a collaborative, multidisciplinary team dedicated to supporting individuals experiencing houselessness through comprehensive, culturally responsive services. The team serves as client advocates. They support individuals seeking medical care, assist with landlord communication, coordinating community events, and help meet basic needs.
Pathways Program
Our Pathways Program employs three multilingual Community Health Workers who specialize in resource navigation and housing assistance. Referrals are received through three primary channels: direct transfers from the Community Health Hub at PeaceHealth SW Medical Center, referrals through the Outreach Program, and self-referrals generated through proactive community engagement by our team.
Community Health Hub
Located at PeaceHealth SW Medical Center, the Community Health Hub serves as a centralized access point where diverse community partners connect with patients prior to hospital discharge. Social workers throughout the hospital submit referrals, which are then routed through the Hub to the most appropriate case manager based on individual client needs. As a key partner in this initiative, SHARE provides housing resources, comprehensive assessments, and community navigation support.
